Sports Requalifications
EVERYONE, 7th – 12th grade, must complete the requalification process in order to tryout for fall sports.
Below is a checklist of what will be needed to complete the requalification process. (first 3 items must be documented for ALL students).
• Sports Clearance Form signed by parent within 30 days of first practice
• Monroe County Physical Examination Form WITHIN PAST ONE YEAR, dated & listing level of participation
• Proof of tetanus shot within past 10 years
• Self med release form, if student carries an inhaler or an Epi-Pen (on back of Physical Examination form)
• Physician's release form, if student has been excused from sports or gym due to an injury or an illness
• Glasses, if prescribed, for the vision test
Requalifications will take place in the band room as follows:
Thursday, August 5th: 9:00 - 2:00 Wednesday, August 11th: 9:00 - 2:00
If it has been more than one year since your student's last physical or more than 10 years since the last tetanus shot, you should schedule a doctor's appointment now. Nurses may not interview students who don't have a current sports physical on file and students without a nurses interview cannot be medically cleared for tryouts, so it is important that documentation of a current sports physical be provided. Bear in mind that many doctor's offices close for vacation during the summer. To avoid disappointment in the fall it is best to be sure the medical paperwork is in order now.
Students enrolled in summer school should arrange now to take off from class on one of the requalification dates. Parents need not attend requalification with the students; however a parent must have signed the Sports Clearance Form. Students must appear in person for sports requalifications.
STUDENTS FAILING TO APPEAR FOR MEDICAL CLEARANCE MAY NOT PARTICIPATE IN PRACTICES OR TRYOUTS.
The starting date for JV, Varsity & freshman sports will be Mon, August 16. Modified football will begin August 30 and modified soccer will begin when school opens on Sept 8. Requalifications should be done on the designated dates.
